Claim Submission Check List

SPECIAL POINTS FOR THE OPERATORS TO BE NOTED FOR:
A. CLAIMS FOR PROVISION OF RURAL COMMUNITY PHONES

A copy of the first bill is to be submitted in support of claim for RCPs   installed.
Claims should be only for list of Villages for provision of RCPs as approved by DOT.

B. CLAIMS FOR PROVISION OF RURAL HOUSEHOLD DELS BETWEEN 1.4.02 AND 31.3.05

Bill Summary as specified in Agreement in support of Claims.
The number of permanently closed DELs reflected in the claim statement should be from out of the DELs installed on or after 1.4.02.
Opening Balance of DELs should be DELs existing as on 1.4.02.
In Attatchment 1/2 to Annexure I(summary and claims statement for front loaded subsidy), the highest number of Rural DELs in any previous Quarter(Column No 10) is to be considered from
1.4.02 onwards.


C. CLAIMS FOR PROVISION OF RURAL HOUSEHOLD DELS (Installed between 1.4.05 and 31.3.07)

The USPs are required to ensure the following points before while submitting their claims:

The Bill Summary is submitted as specified in Agreement in support of Claims.
 The number of permanently closed DELs reflected in the claim statement should be from out of the DELs installed on or after from 1.4.05.
Opening Balance of DELs should be DELs existing as on 1.4.05.
In Attachment 1/2 to Annexure I(summary and claims statement for front loaded subsidy), the highest number of Rural DELs in any previous Quarter(Column No 10) is to be considered from 1.4.05 onwards.

D. AGREEMENT FOR PROVISION OF WIRE LINE BROADBAND CONNECTIVITY IN RURAL AND REMOTE AREAS

A copy of the first bill is to be submitted in support of claim for Kiosks   installed
Claim should be only for BB connections installed in exchanges as specified in the Agreement
Numbers installed in a particular quarter can be claimed in the same quarter or in the subsequent quarter (but not in later quarters).
Computer claim should be only against USO-99 or USO-150 plans i.r.o individual connections. For institutional connections computers can be claimed for any USO approved plan.
CPE subsidy can be claimed up to 63 connections per exchange.
Connectivity subsidy for individual connections can be claimed only up to 31 connections per exchange.
